Levels of Care

*only applies to assisted living

Each assisted living resident’s care level is assessed at move in and then quarterly to ensure the proper level of care is being given to fit the resident’s needs. Should there be a need for a change in level, the Director of Health Services will contact you to hold a care plan meeting. It is our goal to keep each resident as independent as possible for as long as possible.

Level 1: Includes a pendant, monthly vitals checks, and I’m OK checks three times a day. Level 1 residents manage their own medications and personal care. Added benefits include weekly housekeeping and bed linen service. Level 1 also includes your phone and cable at no extra charge.
Level 3: Includes all of Levels 1 and 2, but adds daily care by our QLS’s. They will assist in activities of daily living (ADLs), like dressing, grooming, bed making and escorts to activities and meals.
Level 2: Includes all the above, plus medication administration. Licensed nurses will manage the medication lists, reordering meds, and monitoring. This level also includes showers two times per week by our Quality of Life Specialists (QLS). Pendants may be used for assistance.
Level 4: Is used when a resident needs round the clock care from our QLS’s. In addition, this level is for anyone requiring sliding scale diabetes management, routine oxygen management, daily health monitoring, like daily blood pressure checks, showers more than two times per week, showers which exceed thirty minutes each or 24-hour incontinence management.
